Whether your child has recently received an Autism Spectrum Disorder (ASD) diagnosis or you are exploring ABA services for the first time, we are committed to providing support every step of the way
Before You Start
Information You'll Need
To help us review your child's eligibility for services, please have the following information available when completing the referral process.
1. Current Services
Details about any services your child is currently receiving, such as speech therapy, occupational therapy, or ABA services.
Parent/Guardian Information
Basic contact information for the parent or legal guardian.
Child Information
Demographic information and details about your child's needs and diagnosis.
Insurance Information
Primary and secondary insurance information, if applicable.
Healthcare Provider Information
Information about your child's physician, psychologist, or other providers involved in their care.
Documents to Upload During Referral
Required Documents
Providing documentation during the referral process helps our team complete a more accurate and efficient review.

What Happens After Submission?
Our Enrollment Process
Referral Received
Our intake team securely receives your referral information and uploaded documentation.
Clinical Eligibility Review
Our clinical team reviews records and determines whether ABA services appear appropriate based on your child's needs.
Insurance Verification
We verify benefits, authorization requirements, and available coverage.
Assessment Scheduling
If appropriate, a comprehensive ABA assessment is scheduled.
Treatment Planning & Enrollment
Following assessment and approval, an individualized treatment plan is developed and services are scheduled.

Submission of a referral does not guarantee admission into services.
All referrals are subject to:
- Documentation Review
- Insurance Verification

- Medical Necessity Requirements
- Geographic Service Area Considerations

Families will be asked to complete:
- Consent for Preliminary Review
- HIPAA Privacy Acknowledgement
- Authorization to Obtain and Exchange Information
- Electronic Signature & Date
